ELDERCARE

  • Check in on your loved one
  • Ensure your loved one has a medication system in place and are taking medications correctly
  • Provide resources to safety proof home
  • Provide resources for assistance with meals, transportation, bath aides, and extra senior help

We can check in on your loved one and help them establish a sensible routine which can help elicit good compliance with taking medications. We understand how overwhelming it can feel to care for a loved one. This can give you more time to focus on caring for your loved one in other ways.

END OF LIFE CARE

  • Initiate discussions for end of life decisions including a living will and healthcare power of attorney
  • Provide resources to prepare advanced directives
  • Research palliative care and hospice care options

Advanced care planning is important for your peace of mind and for your loved ones. Having this in place will ensure your loved ones will carry out your wishes if you cannot speak for yourself.

We know your loved one may be struggling, and we want to fill your loved one’s day with joy.

FEES

  • We offer a 20-minute initial phone consultation at no charge to determine preliminary goal of services.
  • We will then schedule a personalized, comprehensive assessment to review your medical history, concerns, and goals. This usually takes one to two hours. Payment for the assessment is required in advance.
  • After this meeting, we will provide you with a personalized action plan formulated to best help meet your goals. A proposal of services will be given at this time. Fees are assessed on an hourly basis.
  • Services are not covered by insurance.
